If account is for a center or practice group:
Your name, email, phone, mailing address.
Your center's name, address.
Lineage the center is affiliated with.
URL of the center's website, if there is one.

If account is for an individual:
Your name, email, phone, mailing address.
Name of the center or practice group you are associated with, if any.
Lineage you are affiliated with.
